How do I request building and door access?

To gain access to the locked doors that lead to your respective SMART lab within the Katz building, you can submit a request through the following google form: https://docs.google.com/forms/d/e/1FAIpQLScmb6YzBXV9bRTvEu5mDWnE9CS1IdDQD3umOxkmTxsIPShv1g/viewform
Ensure the correct email is listed at the top of the form. If the wrong email is listed, it can be changed by clicking on the blue ‘Switch account’ button, as seen in Figure 1.
Select the type of request by clicking the drop down menu, as seen in Figure 2, and then selecting one of the available options, as seen in Figure 3.
Select your supervisors name from the list provided. If their name is not listed, select the ‘Other:’ option and type their name in the text field, as seen in Figure 4.
Enter your first and last name in the text fields, as seen in Figure 5.
Enter your employee or student ID number that is listed on your ONEcard, as well as your HID/proximity number, as seen in figure 6 and 7, respectively.
Select if you are a student or staff, as seen in Figure 8. Only students and staff are able to request access because a ONEcard is required. If you are not a student or staff member, you can contact the lab to gain access to the space upon arrival of the locked entry.
Enter the email address you want to be contacted with, as seen in Figure 9. You can also provide your phone number incase we can't get a hold of you by email.
Enter the length of time you wish to have the access for, as seen in Figure 10. If you end up requiring access for a longer or shorter period than expected, you can submit a new request through this form to make changes to the dates.
Select which building(s) and room location(s) you require access to, as seen in Figure 11.
Select which times you require access. Intermittent access will only be granted in special circumstances.
If there are any problems or concerns with the submitted request our staff will reach out for clarification or to explain any issues with the request.
